Aim of Assessment:
The purpose of this holistic health assessment task is to help you increase your knowledge and skills in
performing an assessment on an adult person focusing on vital signs measurements. You will need to
demonstrate an insight into how differences in vital signs measurements may commonly indicate a person’s
wellbeing. Satisfactory completion of this assessment item will assist in enabling you to demonstrate that you
have met the learning outcomes 1-9.

Case Study:
Tony Milano is visiting the General Practitioner (GP) clinic for a check- up. You are the Practice Nurse who will
interview Tony for his health assessment before he sees the GP.

Tony is a 49yr old male who is widowed and lives alone since his wife passed away from cancer 2 years ago. Tony recently retired 2 months ago as his job was physically demanding as a builder and informs you that there is a strong family history of cardiovascular disease and hypertension. Tony mentions that he has felt “run- down, lethargic and has had a runny nose for the last couple of days ‘’. Upon assessment you notice Tony’s skin is warm to touch. Further questioning highlights that Tony does not cook at home, as it is easier to buy takeaway food. This is usually deep fried food 3 to 4 days a week. He drinks very little water and instead drinks soft drinks and drinks up to 8 cups of coffee a day. Tony also states that he does not do any regular exercise and watches television all day.

You take Tony’s set vital signs which are:
Temperature 37.9 C
HR 90bpm
BP 145/78mmHg

Using the case study and the vital signs above answer the questions below. You are required to refer to the supplied resources on the vUWS site to support each of your answers using in-text reference citations that are correctly cited in your reference list according to APA guidelines at the end of your work.
